This service consists of providing activities for the recovery of autonomy in temporary accommodations when, after hospitalization, the return home of an elderly person is still possible but leaves the patient feeling insecure and uncertain. The objective of the stay in the unit is to optimize the potential for rehabilitation and recovery in order to help her return home and to be as independent as possible in pursuit of her daily life and domestic activities.
Who can benefit
Patients admitted to the unit have a stable medical condition. They are therefore able to collaborate in achieving the objectives of their interdisciplinary intervention plan, to participate actively in looking after their hygiene, to function as if they were at home, that is, to get up in the morning, get dressed, eat their meals in the dining room, get around the unit and outside according to their level of physical autonomy, and to participate in the self-medication program as soon as possible.
The care personnel support and stimulate the patient in her daily activities in order to consolidate the work done with the rehabilitation team.
