Description

The Homecare Support services of the CSSS des Sommets are intended for every person, of no matter what age, with a temporary or persistent disability, the cause of which may by physical, psychological or psychosocial, and who must receive all or some of the necessary services at home. The support services are also available to that person’s caregivers.

Homecare support is offered when the following conditions are fulfilled:
• The need for support is certified by a professional evaluation, on the basis of needs expressed by the person and her caregivers
• The person and her caregivers accept to participate in the decision-making process and to receive the required services. In certain specific circumstances, either the person or a loved one can give this consent.
• The home is judged adequate and safe.

The services available are: medical services, nursing care, psychosocial services, respiratory therapy, nutrition services, basic rehabilitation services (occupational therapy and physiotherapy), community organization services, access mechanism for residential facilities, homecare services, hygiene services, care and assistance services, support for daily activities, technical support services (equipment loans), services to caregivers, respite and alternate respite services, babysitting, psychosocial support, etc.

To avoid hospitalization or to allow a rapid return home, certain intensive services will be provided, such as: nursing care (up to 3 times a day); homecare (more than 3 times per week); physiotherapy (up to 3 times a week); respiratory therapy (maximum of one visit per day); nutrition (maximum of one visit per day in a limited and very short-term manner, for example: tube feeding in the home) and direct benefits – SISAD (intensive homecare support services).

The services can be provided:
• In the person’s home if he is confined because of the state of his physical or mental health or when it is more appropriate in the clinical plan to provide some or all required services in the home.
• On an outpatient basis when the person is confined to home but when it is more appropriate in the clinical plan to provide some or all of the services by going to the institution or to another place designated by a professional member of the team.

Who can benefit
For clients presenting many profiles: loss of autonomy related to aging, physical disability, short-term physical health problems, post-hospitalization, post-surgery, palliative care and public health.
